It uses the time selection, un-linked from the loop selection, which enables you to use it within your current loop (great for beat building :D)
I use the ALT+Right-click to select the time and item(S) I want to duplicate, then ctrl+d. If you only selected a portion of the item, then what ever is within that time selection (and selected) is duplicated.

This is cool but it crashes Reaper 3.76
Repro:
select a portion of the loop with alt+RCLICK
CTRL+D to dupe that selection
select the original main portion of the loop
CTRL+D several times
